8:00 AM - 5:00 PM

Upon Enrollment: The following are required to enroll your child:

* Completed enrollment forms

* Immunization record

* Health Statement signed by a physician

* If your child is four years of age or older-vision & hearing screening

* Registration Fee: $85.00 upon enrollment.

(one-time annually, non-refundable)
